ACA coverage offers

ASU offers the following job-based benefits to eligible faculty and staff and their eligible dependents as mandated under the Patient Protection and Affordable Care Act:

Enrollment steps

To elect coverage during open enrollment

  1. 日本一本道a不卡免费The ASU ACA Compliance team will send an email to your ASU email address.

  2. To assist you and your family with coverage decisions, use the benefits open enrollment worksheet.

  3. You must complete online open enrollment during the specified time period. Important: If you fail to complete enrollment during the specified time period, your benefits will automatically be declined. Your next opportunity to enroll will be during the next open enrollment period unless you have a qualifying life event (QLE).

  4. Coverage elected during open enrollment is effective the following January 1. Exception: Aetna Life plans that require underwriting approval may have a later effective date.

  5. 日本一本道a不卡免费Coverage continues through the remainder of the calendar year as long as you are an ASU employee.

  6. After January 1, use My ASU to designate your life insurance beneficiaries.

 To elect coverage when newly eligible

  1. 日本一本道a不卡免费The ASU ACA Compliance team will send an email to your ASU email address.

  2. You must submit a completed benefits enrollment/change form with required supporting documentation (if applicable) to HR Benefits during your 30-calendar-day enrollment period. Important: Failure to complete enrollment during your 30-calendar-day enrollment period will result in an automatic declination of benefits.  Your next opportunity to enroll will be during the next open enrollment period unless a qualified life event occurs.

  3. 日本一本道a不卡免费Depending on the reason you became eligible, coverage may be effective the first day of the pay period in which the first day of the month occurs, on the rehire date, or on the FTE change date.

  4. 日本一本道a不卡免费Coverage continues through the remainder of the calendar year as long as you are employed by ASU.

  5. After coverage becomes effective, use My ASU to designate your life insurance beneficiaries.

To change coverage when a qualified life event occurs

Submit a completed benefits enrollment/change form with required supporting documentation to HR Benefits within 30 calendar days of the event’s effective date. 
日本一本道a不卡免费Remember: Changes to your benefit elections can be done:

  • 日本一本道a不卡免费During Open Enrollment (which occurs in November each year with a January 1 effective date) or

  • Within 30 calendar days of a Qualifying Life Event (QLE), which is an event defined by the Internal Revenue Service in Section 125 that allows changes to be made to benefit elections outside of open enrollment.

日本一本道a不卡免费The ACA does not require you to enroll in benefits offered by ASU. However, whether you enroll or decline these benefits, you will be automatically enrolled in a university-paid basic life insurance policy equal to your annual salary.

If you decline coverage, please formally decline in writing  or submit a completed benefits enrollment/change form declining all coverage to HR Benefits within the 30-calendar-day enrollment period. Failure to elect benefits or provide written notice of your decision within this timeframe will serve as an automatic declination.

If eligibility is met following the initial measurement period, ACA benefits continue for one year (the stability period). 

  • Eligibility after one year is then determined for the next year following the standard measurement period. 

  • If eligibility is not met following the standard measurement period, coverage may terminate at midnight on the last day of the pay period in which your last day of eligibility occurs.

  • 日本一本道a不卡免费You will be given the opportunity to continue your medical, dental and/or vision coverage under the federal regulations. Information will be mailed to your home by the Arizona Department of Administration Benefit Services Division.

If eligibility is met following the standard measurement period, ACA benefits continue through the end of the calendar year. 

  • 日本一本道a不卡免费Eligibility in the next calendar year is then determined during the standard measurement period. 

  • Coverage may terminate at midnight on the last day of the pay period in which the last day of the calendar year occurs. 

  • You will be given the opportunity to continue your medical, dental and/or vision coverage under the federal regulations. Information will be mailed to your home by the Arizona Department of Administration Benefit Services Division.

If you fail to pay your share of the premium cost by the due date, ACA benefits will terminate at midnight on the last day of the pay period in which premiums were paid in full. See Premium Costs section below.

If ASU employment terminates:

  • Coverage terminates at midnight on the last day of the pay period in which your last day worked occurs.

  • You will be given the opportunity to continue your medical, dental and/or vision coverage under the federal regulations. Information will be mailed to your home by the Arizona Department of Administration Benefit Services Division.

Under the ACA, ASU is required to make health care premiums affordable so you will be billed only for your share of the benefit premium costs. 

  • Your share will be automatically withheld from your biweekly paychecks. 

    • If you don’t receive a biweekly paycheck, we will send an invoice to your home address listed in My ASU.   

    • If you fail to remit premiums for your share of the costs by the due date, your coverage will terminate at midnight on the last day of the pay period in which premiums were paid in full. Further, you will forfeit your right to continue coverage under federal regulations.

  • Arizona State University Benefits Office (if regularly scheduled to work 20 or more hours per week for at least 90 days in a non-student position)
