No, you cannot enroll as a single subscriber and be enrolled as a dependent on your spouse's policy simultaneously. If you do enroll in that manner, no refunds will be made for the employee contributions.

日本一本道a不卡免费Yes. As mandated under the October 4, 2010, federal healthcare reform legislation, you may add them to your medical, dental and/or vision coverage only. This coverage is limited to your children up to age 26 only; it does not include their spouses or children. To cover grandchildren, you must obtain and submit legal Letters of Guardianship to HR Benefits Design and Management.

  • Log into My ASU

  • Go to My Employment > Benefits > My Benefits Summary.

  • For future dated benefits, input the future Date and Click Go.

You should see a list of all of your benefits elections. If you do not, please contact HR immediately via email or ASKHR.

日本一本道a不卡免费In accordance with Section 125 of the Internal Revenue Code and the Arizona Board of Regents Health and Dependent Care Cafeteria Plan, changes to your benefit elections may be made: 

  • 日本一本道a不卡免费During the annual benefits open enrollment period that occurs in October/November each year effective the following January 1, or

  • Within 30 calendar days of a qualified life event (QLE). A QLE is an event defined by Section 125 that allows changes to benefit elections outside of the annual benefits open enrollment period.

Additional documentation is required for the following:

  • Child with different last name: Birth certificate or court order

A Social Security number is required for all covered dependents. 日本一本道a不卡免费If your dependent is not eligible for a SSN due to his/her Visa status, you must submit substantiating documentation.

Completion of enrollment means submitting of a completed Benefits Enrollment/Change Form and supporting documentation to the HR Benefits Office within 30 calendar days of hire/eligibility/QLE date. After the 30-calendar-day period, elections may be made only during the annual benefits open enrollment period unless a QLE occurs.

日本一本道a不卡免费Any change of election must be consistent with the QLE. For example, if an employee marries, the employee may add the new spouse or terminate coverage and become a dependent on the spouse's plan.  However,  the marriage would not allow the employee to change networks within ASU's plan.

Before accessing services, please verify the effective date of coverage related to the change by logging in to My ASU > My Employment > Benefits.

日本一本道a不卡免费This is a federal requirement in accordance with the Mandatory Insurer Reporting Law (Section 111 of Public Law 110-173), which took effect January 1, 2009.

To update a dependent/beneficiary social security number:

Important

  • Newborn social security numbers must be submitted with a Dependent Data Update form within 60 days of the date of birth (only required if enrolled in ASU benefit plans).

  • Name, Date of Birth for existing dependent/beneficiaries cannot be modified through My ASU once entered and saved.

  • Arizona House Bill 2002 – If hired or newly-eligible on or after September 12, 2013, coverage is effective the first day of the pay period following completion of enrollment.

  • Arizona House Bill  1614日本一本道a不卡免费 – If hired or newly-eligible on or after July 20, 2011, coverage is effective the first day of the pay period following a 90-day waiting period. 

  • Autism Coverage日本一本道a不卡免费 – Prohibits group health plans from denying coverage, imposing dollar limits or charging higher deductibles/copays based solely on the diagnosis of autism spectrum disorder. It also requires plans to cover the cost of behavior therapy up to $50,000 per year for a child up to age 9 and $25,000 per year for a child age 9-16.

  • HEART Act 日本一本道a不卡免费– In accordance  with Heroes Earnings Assistance and Relief Tax Act of 2008 (the HEART Act), qualified military reservists who participate in a flexible spending account (FSA) program may withdraw FSA funds (and avoid the use-it-or-lose-it rules) when they are called to active duty for 180 days or more, or for an indefinite period. The withdrawal must be made during a period beginning on the day the reservist is called to active duty and ending on the last day of the coverage period of the FSA plan that occurs during the period of active duty.

  • Michelle’s Law – A new federal law allows continued coverage for seriously ill college students. A college student will be able to maintain health plan eligibility for up to one year after full-time student status is lost due to a medically necessary leave of absence from school.

  • COBRA Premium Assistance – Under a new federal law, a person involuntarily terminated between September 1, 2008, and February 2010, may be eligible for COBRA premium assistance. Under this program, the individual pays 35% of the COBRA premium and the federal government subsidizes the remaining 65%.

  • Genetic Information Nondiscrimination Act (GINA)日本一本道a不卡免费 – Under a new federal law, group health are prohibited from adjusting premiums of contributions amounts for a group on the basis of genetic information. A health plan also is prohibited from requiring an individual or his/her family member to undergo a genetic test, although the plan may request that a voluntary test be taken for research purposes.
