Hazardous Waste Pickup Request - Used to request removal of hazardous waste.

Laboratory safety

  • Lab-specific training plan - PI’s and managers to use to document lab-specific training requirements.

  • Minors in lab request - Risk Management will return a Parental Release Form after receiving the request form.

  • NFPA chemical labels - Use with Avery 5160 type label, used to label chemicals in laboratories.

  • Prior approval form - Before obtaining a new hazardous chemical a prior approval form this must be filed with EHS.

Personal Protective Equipment

  • PPE hazard assessment日本一本道a不卡免费 - All managers and supervisors must survey the work areas and activities under their control to determine where PPE may be required.

  • Respiratory protection evaluation - Instructions for employees and non-Facilities Development and Management employees.

Confined space entry -  A permit is required to enter a confined space that may have a hazardous atmosphere or contains recognized serious safety or health hazard. For more information email Environmental Health and Safety, or call 480-965-1823. 

 

  • Special Event Permit - File the event with the Special Event Registry at least 30 days before the occasion.  Submit supplemental paperwork to the ASU Fire Marshal at least 10 working days prior to the event to avoid additional fees.

  • State Fire Marshal plan review permit application日本一本道a不卡免费 - All ASU building renovations and new construction require a state fire marshal plan review permit. The application must be submitted by the contractor or the project manager with construction plans.

  • Emergency evacuation drill - Submit this report to the University Fire Marshal's office via email日本一本道a不卡免费 or fax at 480-965-0736.

The radiation training registration form is required for the following:

  • 日本一本道a不卡免费Laser - Class is for anyone working at or visiting ASU who uses Class IIIb or IV lasers. Prior to taking this class, the user must have a baseline eye exam.

  • 日本一本道a不卡免费Radioactive material training - Class is required for people working at or visiting ASU who procure, use or dispose of radioactive materials.    

  • Radiation producing equipment training - Class is required for people who want to procure, or need access to ionizing radiation producing equipment

  • Incident report日本一本道a不卡免费 - Report any accident that involves a vehicle, injury, property loss or near miss that involves ASU facilities or property whether or not a claim is filed. 

  • Indoor air quality - Asbestos - Used to request removal, repair, encapsulation, testing or evaluation for known or suspected asbestos contamination.

  • Working alone checklist - Used to make sure standard operating procedures, training, safe working procedures, and communication procedures for people working alone are in place.
