Insurance waivers

Departments must utilize the Speaker-Artist-Performer Agreement template located on the Office of General Counsel’s webpage for short-term engagements of one week or less.  The agreement requires the Speaker-Artist-Performer to carry commercial general liability insurance in an amount of not less than $1,000,000, single limit, against claims for bodily injury, death, and property damage occurring in connection with the Event and the Presentation.

日本一本道a不卡免费For Speakers-Artists- Performers that do not carry the required insurance, a department may request a waiver of insurance from Risk and Emergency Management by completing a . A matrix has been created to help departments evaluate whether a waiver of the insurance requirements should be requested, see number two below.

The department must include with the Request for Waiver of Insurance form a clear and concise description of the scope of work that is detailed enough to understand exactly what the individual will be doing for ASU.

Departments will need to certify on the Request for Waiver of Insurance form the following are true:

  1. The Speaker or Performer is not represented by an agent for the engagement; the Speaker or Performer is an individual, not a company; and the total payment to the Speaker or Performer will not exceed $10,000.

  2. The Speaker or Performer, and any events related to the Speaker’s or Performer’s engagement at ASU, do not include any of the following:

    1. 日本一本道a不卡免费Props that pose a significant risk to persons or property, such as animals, fireworks, smoke, hazardous materials, fire, etc.

    2. Equipment that poses a significant risk to persons or property, such as trapezes, heavy equipment including motors, vehicles, mini motorized vehicles or similar types of equipment.

    3. Installation of large amounts of wiring/cabling, not standard extension cords, or cabling/wiring that extends into or within very close proximity to the audience.

    4. Any other significant risk of injury to persons, other than Speaker or Performer, or property.

    5. The need to connect to ASU network or database other than ASU guest WIFI.

For all other requests for a waiver of insurance requirements, please send an email to ASU Insurance with a clear and concise description of the scope of work that is detailed enough to understand exactly what the individual will be doing for ASU and why a waiver is being requested.
